Remuneration: Circa £50,000 - £110,000

Description: Part Time/Full Time Performer Dentist required 2-5 days per week to work for a busy practice near Swindon, Wiltshire.

1. £14 per UDA 2-3000 UDAs
2. 40%/50% Private
3. Locum to Permanent an option
4. 4 Computerised surgeries & software
5. Equipment: Digital x-rays, Rotary Endo & Apex Locator, iTero Scanner, OPG, CBCT, Microscope
6. Hygienist & Therapists on site
7. Qualified nurses
8. PM/Treatment coordinator on site
9. Close to transport links, in a Town Centre
10. Available now, negotiable

1. Full NHS Performer Number if applicable
2. Local Area Team confirmation
3. GDC certificate
4. UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
5. Indemnity certificate
6. Passport
7. Enhanced DBS (within the last 12-36 months)
8. Clinical references covering places of employment over past 2 years (minimum 2 referees)
9. Degree certificate
10. CPR/AED/Medical Emergencies certificate dated within the last 12 months
