Description:

Full or part-time Performer Dentist required for 2-4 days per week (FLEXIBLE) in a busy, mixed, INDEPENDENTLY OWNED practice in Stockport, Greater Manchester.

Compliance requirements:

1. Full NHS Performer Number
2. Local Area Team confirmation
3. GDC certificate
4. UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
5. Indemnity certificate
6. Passport
7. Occupational Health Clearance
8. Prison Vetting – if possible
9. Sys 1 access/card – if possible
10. Enhanced DBS (within the last 12-36 months)
11. Clinical references covering places of employment over past 2 years (minimum 2 referees)
12. Degree certificate
13. CPR/AED/Medical Emergencies certificate dated within the last 12 months

15. PIN number
