Description: Part-time Performer Dentist required for 2-5 days per week in an independently owned mixed practice near St Ives, Cornwall.

1. 2000-7500+ UDAs available
2. Negotiable UDA rate depending on experience
3. 50% split on private
4. 50% split on lab bills
5. Fully computerised surgeries
6. Digital x-rays
7. Rotary endo
8. Full support team
9. 20 minutes from Penzance; 30 minutes from Redruth
10. Available now, start date negotiable

Compliance requirements:

• Full NHS Performer Number
• Local Area Team confirmation
• GDC certificate
• UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
• Indemnity certificate
• Passport
• Enhanced DBS (within the last 12-36 months)
• Clinical references covering places of employment over past 2 years (minimum 2 referees)
• Degree certificate
• CPR/AED/Medical Emergencies certificate dated within the last 12 months
