Remuneration: Circa £80,000

Description: Full or Part-time Performer Dentist required for 3-4 days per week in a busy, mixed, well-established, and INDEPENDENTLY OWNED Family practice near Telford, Shropshire.

1. £13.50 + depending on skillset & experience
2. 4000-6000 UDAs available
3. 50% split on Private & Labs
4. Practice provides predominantly General Dentistry but new skills welcome!
5. Hours 8am-5.15pm Flexible
6. Well Established list
7. Fully computerised Modern surgeries with air conditioning
8. 3 surgery practice, 2 Associates
9. SOE Software
10. Clinical Freedom
11. Digital X-rays, Rotary endo, Intraoral Cameras
12. Full support team including a Hygienist
13. Parking on Site
14. Available Immediately, start date negotiable

Compliance requirements:

1. Full NHS Performer Number if applicable
2. Local Area Team confirmation
3. GDC certificate
4. UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
5. Indemnity certificate
6. Passport
7. Enhanced DBS (within the last 12-36 months)
8. Clinical references covering places of employment over past 2 years (minimum 2 referees)
9. Degree certificate
10. CPR/AED/Medical Emergencies certificate dated within the last 12 months
