Remuneration: Circa £100,000+

Description: Part-time or Full-time Dentist required for 3 days per week in a busy, well-organised practice in Sherbourne, Dorset.

Compliance Requirements:

1. Full NHS Performer Number
2. Local Area Team confirmation
3. GDC certificate
4. UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
5. Indemnity certificate
6. Passport
7. Enhanced DBS (within the last 12-36 months)
8. Clinical references covering places of employment over past 2 years (minimum 2 referees)
9. Degree certificate
10. CPR/AED/Medical Emergencies certificate dated within the last 12 months
11. IRMER
12. PIN number
