Remuneration: Circa £85,000+

Description: Part OR Full time Performer Dentist required for 2-5 days (flexible) in a busy, mixed, well established INDEPENDENTLY OWNED practice near Ellesmere Port, Cheshire.

1. £13-£14 per UDA depending on experience
2. Good private potential at the practice
3. Practice are happy for the dentist to take a smaller target with more focus on private if required
4. 50% split on Private & Labs
5. 7.5-8 hours between 8.30am and 5.30pm
6. 5 surgery modern dental practice with ambidextrous chairs
7. Digital x-rays with patient screens, Rotary Endo, R4 software
8. Facial aesthetics, composite bonding advantageous
9. Full support team including a Hygienist/Therapist
10. Existing, well maintained list
11. Strong admin, management team with excellent nursing support
12. Parking available at the practice
13. 15 minutes to Chester, 25 minutes to Birkenhead
14. Position to start ASAP or notice periods taken into consideration

1. Full NHS Performer Number if applicable
2. Local Area Team confirmation
3. GDC certificate
4. UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
5. Indemnity certificate
6. Passport
7. Enhanced DBS (within the last 12-36 months)
8. Clinical references covering places of employment over past 2 years (minimum 2 referees)
9. Degree certificate
10. CPR/AED/Medical Emergencies certificate dated within the last 12 months
