Description:

Part Time or Full Time Performer Dentist required 2-5 days per week for a mixed practice in Ballymoney, Northern Ireland.

1. 45% on NHS and private
2. 3 Fully computerised surgeries

2. Local Area Team confirmation
3. GDC certificate
4. UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
5. Indemnity certificate
6. Passport
7. Enhanced DBS (within the last 12-36 months)
8. Clinical references covering places of employment over past 2 years (minimum 2 referees)
9. Degree certificate
10. CPR/AED/Medical Emergencies certificate dated within the last 12 months
