An exciting opportunity has arisen for a band 4 Assistant Practitioner or Nurse Associate in Barking and Dagenham.
We are seeking a motivated and dynamic Band 4 Assistant Practitioner or Nurse Associate for this exciting opportunity to work within the Community Health and Social Care (CHSCS) teams within Barking and Dagenham. The CHSCS teams are a forward-thinking and innovative service that is community-based, working in a fully integrated way with physiotherapists, occupational therapists, district nurses, and community matrons to ensure a holistic approach to care.
You will be expected to carry out clinical visits to patients in their own homes and residential homes under the direction of the registered nurse.
The successful applicant must be flexible, a good team worker, used to working under pressure, have excellent interpersonal and IT skills, and be able to communicate effectively with patients, carers, and other members of the MDT.
We are interested in hearing from both Assistant Practitioners and Nurse Associates with proven clinical and communication skills to work as part of our friendly, supportive, and innovative team. Previous experience in a community setting in the NHS is highly desirable but not essential.
The successful applicant will demonstrate the ability to utilize a range of skills and experience gained in clinical practice to assist the registered clinician in undertaking comprehensive and holistic assessments.

Starting with NELFT: NELFT places a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks, which will include mandatory training, systems training, and the allocation of equipment. As part of the process, new starters will have the opportunity to meet the executive team, senior managers, and attend drop-in sessions focusing on engagement, health and wellbeing, and key processes. The induction will be held at our head office in Rainham, Essex.
Probationary Period: This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently partway through a probationary period or currently a bank member of staff).
High Cost Area Supplement - London: This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum).
