Job Overview

The Therapy Team on Sneyd Ward deliver care and rehabilitation over a 7-day service to adults admitted on to the ward with a new onset Stroke from across the local acute portals. The team focus on ensuring the service users are supported physically, cognitively and emotionally within a biopsychosocial rehabilitation model using client centred practice. As a rehabilitation assistant practitioner, you will be working as part of an inter-disciplinary team where you will need to work alongside all disciplines with a proactive approach to service users recovery embedding quality at its core. Under the direction of registered physiotherapists and Occupational Therapists, you will be involved in the implementation and evaluation of SMART goals to ensure service users under your care are provided with a collaborative, effective and evidence-based service to facilitate their discharge from an in-patient setting to the community. As a rehabilitation assistant practitioner, you will also have responsibility for your own allocated caseload under the supervision of registered therapy staff.

Detailed Job Description And Main Responsibilities
  • Have an awareness of the local and national drivers influencing Stroke Rehabilitation.
  • To follow the RCOT and CSP Standards and Guidelines for Stroke, including all other guidance affecting Allied Health Professional services.
  • Receive information relating to patients diagnosis, treatment and rehabilitation, action any response within your scope of practice, and feedback to Registered staff.
  • Liaise with family members as required to meet the needs of the patients.
  • To be responsible for your own practice within your skills remit and competence of the Rehabilitation Assistant Practitioner.
  • To implement treatment programmes autonomously to meet the patients needs under the supervision of a Registered Therapist within the service, facilitating the intervention, monitoring the individuals episode of care and progressing this intervention as appropriate.
  • To be able to identify SMART goals for patients on your allocated caseload and sign off goals that have been met.
  • To be responsible for the safe and competent use of equipment by patients and staff.
  • To recognise the importance of risk assessments as appropriate with Trust procedures.
  • To be involved in supporting non-complex patients through their hospital admission to meet service demand and be actively involved in discharge planning including goal setting meetings, board rounds, case conferences, family meetings, ordering equipment from Medequip, completing referrals (e.g. community stroke team, wheelchair referrals, stroke association, olive branch), environmental risk assessments.
  • To be responsible for maintaining accurate and comprehensive records in line with Trust standards of practice.
  • To be responsible for your own personal and professional development, fulfilling the requirements of the post in meeting the requirements of lifelong learning and engaging in continuing professional development.
  • To promote the involvement of patients and carers in the service worked.
  • To be involved in supporting and training of band 3 therapy staff, or other staff within the ward MDT in relation to areas within your scope of practice.
  • To report issues with service provision/patient care to line manager.
  • To support with feedback of figures for the Stroke Specific National Audit Programme (SSNAP) to local data collectors, and support with any other ongoing audits.
  • Undertake any other duties required by Team/Clinical Lead and Specialist Services Manager which are commensurate with the post.
  • To work within service model demands; ability to undertake bank holiday; weekend and shifts patterns with lone working as required.
  • Car driver is desirable to support with service demands and some community working, where appropriate.

Experience

  • Essential criteria: Sound knowledge of a range of clinical presentations and how to escalate concerns.
  • Desirable criteria: Previous experience of working in a community/Hospital setting.
