Detailed job description and main responsibilities

Clinical / Direct Care Practice

The post holder will:

  1. Be responsible for the provision of nursing care on the ward 24 hours a day.
  2. Be accountable for nursing practice, taking every opportunity to sustain and improve, ensuring high standards of care are given to all patients, monitoring this through spot checks and clinical quality indicators outcome measures.
  3. Be a clinical expert recognising deterioration in clinical conditions and initiate a rapid and appropriate response.
  4. Practise clinically, leading, co-ordinating and supervising nursing practice. Ensuring that all patients have their needs assessed, that programmes of care are developed to meet these needs and are delivered in accordance with agreed policies and procedures.
  5. Establish and maintain effective personal relationships with patients and relatives and ensure that the individual's physical and psychological needs are identified.
  6. Communicate with patients at the appropriate level of understanding about issues regarding their medical condition whilst maintaining patient confidentiality. Ensure that they understand the rationale for their care and its consequences and alternatives. Record details of any information given, always maintaining patient confidentiality.
  7. Be accountable for identifying the patient's health education needs and collaboratively implementing programmes of health promotion according to the diagnosis.
  8. Use specialist knowledge to assist patients to give Informed Consent by providing information about their condition, and planned procedure, benefits, risks, complications and effects of the intended procedure, and any alternative treatments.
  9. Ensure patient needs are met by working collaboratively with other professionals and agencies. This includes having a wide knowledge of all the services offered and referring to other health care providers as appropriate.
  10. Act as an advocate for the patient in treatment decisions and choices.

Education and qualifications

Essential criteria

  • Registered Nurse
  • Post registration nursing course in speciality
  • Recognized teaching qualification

Desirable criteria

  • Degree or ability to think and practice at degree level with extensive experience
  • Leadership qualification

Experience

Essential criteria

  • Substantial experience at Band 6 level
  • People management experience
  • Effective clinical leadership and experience in the management of change
  • Experience of working effectively as part of a multidisciplinary team
  • Experience of caring for couples experiencing pregnancy loss
  • Experience of formal and informal teaching
  • Ability to recognise when help or escalation is needed

Desirable criteria

  • Recent experience of data collection and audit

Skills and knowledge

Essential criteria

  • Excellent written, verbal communication and interpersonal skills
  • Sound knowledge of the principles and practice of specialist care
  • Computer literate: able to use word, excel, and manage databases
  • Ability to lead change / innovations in specialist practice
  • Able to critically analyse and apply research to the clinical setting
  • Organisational / prioritisation skills
  • Experience of involvement with safeguarding concerns
