Supportive Outreach and Engagement Worker (Alcohol Care Teams)

A new opportunity has arisen to join the MFT Alcohol Care Team (ACT). This is an exciting time to join the team, as it expands and develops within the hospitals and the wider trust.

The Supportive Outreach and Engagement Worker position has been created to support individuals accessing secondary emergency care services, providing a wider, holistic, and multi-agency approach to case management.

Facilitating integrated alcohol care between secondary, primary, and community services for acute alcohol frequent hospital attendees.

This role will be primarily community-based; however, you may at times be requested to work at one of the 3 Alcohol Care Team locations - Manchester Royal Infirmary, Wythenshawe Hospital, or North Manchester General Hospital.

The focus of the role would be working with high intensity users of the trust, who present to the Emergency Departments or are admitted in relation to alcohol misuse. Ensuring effective implementation of care pathways and being the interface between community substance misuse services and the Alcohol Care Teams. Providing the extension of care beyond discharge, to support individuals who may be resistant to services, hard to engage, or treatment naïve at the point of discharge. The post holder will attempt to re-engage individuals who have dropped out of treatment or detached from services.
