An opportunity has arisen for an Advanced Social Work Practitioner in the Salford Hospital Discharge Team.


Salford is now looking to recruit an experienced Advanced Social Work Practitioner to help us deliver our vision. This role requires a passion for social work, enabling effective integrated working, promoting independence through reablement approaches, and safeguarding adults in Salford. You will be responsible for carrying out social work duties that require the highest levels of skills, knowledge, and professional expertise. You will assist the Team Manager in ensuring the delivery of effective, safe integrated services for adults through the promotion of high standards of practice in assessment and person-centred support planning, being creative to meet individual outcomes.

This will be achieved through developing a skilled and motivated workforce by investing in their continued professional development and ensuring their practice is current and of a high standard.


Main duties of the job

You will be a skilled, effective practitioner acting with considerable autonomy. You should have in-depth knowledge and skill in more than one field of practice, which may have been developed through formal post-qualifying training and development activity. You should be an experienced practice educator and ASYE assessor, supporting student social workers and newly qualified social workers. You will need to have the capability to integrate professional knowledge, the views of people using services, and research/evaluation evidence routinely in your practice and be able to support the team to do so. You will need to demonstrate expert and effective practice in complex situations, assessing and managing higher levels of risk while promoting enablement and self-determination. You will need to network and liaise with a wider range of professionals and other colleagues, including at more senior or strategic levels. You will need to manage a complex workload while supporting the team and the team manager. You, as an Advanced Social Work Practitioner, are expected to promote a culture of professional curiosity and contribute to the positive social work identity within the organisation.

  • Registered as a Social Worker with Social Work England.

Qualifications
Essential
  • Recognised Social Work qualification, Diploma in Social Work, CQSW, CSS.

Knowledge, Skills, Training and Experience
Essential
  • You must clearly evidence an understanding of the principles and values underlying the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation.
  • You must clearly evidence an understanding of the integration agenda and the promotion of independence (enablement) model of health and social care.
  • Demonstrate a thorough understanding of personalisation and a person-centred approach.
  • You must clearly evidence that you can maintain accurate records and communicate complex information clearly and concisely both verbally and in written form.
  • You must clearly evidence that you have a clear understanding of the social work role in a multidisciplinary setting.
  • Experience of working with adults/older people.
  • You must clearly evidence your ability to develop effective working relationships with people with lived experience and their carers and show evidence of good assessment practice.
  • Demonstrate the ability to develop effective working relationships with other professionals and colleagues.
  • Possess skills of negotiation, mediation, and de-escalation - seeking constructive approaches to problem solving.
  • Have a good standard of computer skills.

Desirable
  • You have additional qualifications: Best Interest Assessors, AMHP, Practice Educator, Post Qualifying.
  • You can show knowledge of relevant Welfare Rights Legislation.
  • You can evidence an innovative approach to the development of new methods of working to support people in the community.
