We have some very exciting opportunities to join our Acute Virtual Ward project. This project forms part of a wider plan to improve urgent and emergency healthcare delivery across Plymouth, the South Hams, and West Devon.
We are committed to revolutionising healthcare delivery for frail and elderly patients by launching a Community Frailty Virtual Ward. Our aim is to provide high-quality, patient-centred care in the comfort of their own homes, reducing hospital admissions and length of stay in hospital and enhancing overall well-being through enhanced care coordination.

We are thrilled to introduce a new and influential role within our Acute Virtual ward and our forward-thinking Pharmacy Team. As the Advanced Clinical Pharmacist for the Virtual Ward, you will take the lead in delivering a high-quality clinical pharmacy service, collaborating closely with the Lead Pharmacist for the Virtual Wards. Together, you will drive seamless transitions of care and ensure outstanding outcomes for our patients.
In this role, you will coordinate clinical pharmacy services across the Virtual Wards, employing a holistic approach to medicines optimisation across a variety of healthcare settings. Beyond daily responsibilities, you will be at the forefront of innovation leading clinical audits, spearheading quality improvement projects, and contributing to innovative research that pushes the boundaries of what's possible in patient care.
But that's not all! You will also play a key role in shaping the future of Medicines Optimisation within Pharmacy Services, providing strategic input while maintaining a strong presence within the organisation. Your passion for excellence in pharmacy will help develop and inspire the Virtual Ward Pharmacy Team, as well as healthcare professionals including Non-Medical Prescribers (NMPs), ensuring the highest standards in medicines governance and clinical care.
