We have some very exciting opportunities to join our Acute Virtual Ward project. This project forms part of a wider plan to improve urgent and emergency healthcare delivery across Plymouth, the South Hams, and West Devon.

Our aim is to provide high-quality, patient-centred care in the comfort of their own homes, reducing hospital admissions and length of stay in hospital and enhancing overall well-being through enhanced care coordination.

Main duties of the job

The Role

As an Advanced Clinical Pharmacist, you will be a cornerstone of healthcare delivery and will play a critical role in the multidisciplinary team, covering the Acute Virtual ward that is in operation in our locality.

Here is what you will do

1. Leadership within the Virtual ward regarding medicines and medicines governance.
2. Medicine reconciliation.
3. Structured medication reviews including complex polypharmacy and deprescribing.
4. Prescribing and accurate record keeping regarding medications.
5. Answering medication queries raised by the multidisciplinary team and providing clinical support to the Acute Virtual Ward Team.
6. Discharge facilitation.
7. Work within integrated care across several multi-disciplinary teams including, Mental Health, District Nursing Discharge teams, Acute GPs, and Social Workers.

As the Advanced Clinical Pharmacist for the Virtual Ward, you will take the lead in delivering a high-quality clinical pharmacy service, collaborating closely with the Lead Pharmacist for the Virtual Wards. Together, you will drive seamless transitions of care and ensure outstanding outcomes for our patients.

In this role, you will coordinate clinical pharmacy services across the Virtual Wards, employing a holistic approach to medicines optimisation across a variety of healthcare settings. Beyond daily responsibilities, you will be at the forefront of innovation leading clinical audits, spearheading quality improvement projects, and contributing to innovative research that pushes the boundaries of what's possible in patient care.

But that's not all! You will also play a key role in shaping the future of Medicines Optimisation within Pharmacy Services, providing strategic input while maintaining a strong presence within the organisation. 1. Experience as a practising pharmacist, including experience of working as a member of a Multidisciplinary Team.
2. Experience of networking with other healthcare professionals across the full range of disciplines at all levels.
3. Experience of review and development of pharmacy or medication related policies, procedures and PGDs.
4. Experience of audit work.
5. Experience of delivering education to healthcare staff and patients or carers via workshops, informal discussion or formal lectures.

4. Aware of current healthcare targets, NICE guidance and other national directives and the implications for implementation in primary and secondary care.

Qualifications

Essential

1. Masters Degree or equivalent in Pharmacy.
2. Registered Practising Member of the General Pharmaceutical Council.
3. Specialist knowledge acquired by postgraduate diploma or equivalent evidence of specialist knowledge, competence and achievements in practice in pharmacy.
4. Evidence of Continuing Professional Development and revalidation.

Desirable

1. Postgraduate qualification in clinical pharmacy.

3. Working towards membership of the RPS Faculty.
4. Independent prescriber or willingness to work towards.
