The PCN Advanced Paramedic Practitioner role at Farnham Integrated Care Services (FICS), Farnham Centre for Health, will be a registered Paramedic with post graduate qualifications, including the Independent and Supplementary Prescriber qualification and HCPC registration, acting within their professional boundaries.
The following are the core responsibilities of the PCN Advanced Paramedic Practitioner:
Farnham Primary Care Network (PCN) is a group of practices working together to focus on local patient care. Our work is a collaboration with Downing Street Group Practice, Farnham Dene Medical Practice, and Farnham Park Health Group. We provide a wider range of services to support our patients.
The main purpose of our PCN is to enhance patient outcomes, improve the quality of care, and increase the efficiency of healthcare delivery. As a PCN, we achieve these goals through improved coordination of care, offering a wide range of services, enhancing access to care, managing population health, promoting preventive care, health education, and prioritizing a patient-centered approach.
Our ultimate aim is to deliver better health outcomes for the local population, as well as a reduction in health inequalities. We work alongside Farnham Integrated care services which houses some of our clinicians, the same day service, and enhanced access.
This role will work as part of a multi-disciplinary team of AHPs/GPs/HCAs, providing care for the presenting patients within our Urgent Same Day Care Service at FICS. An established integrated service supporting the GP Practices of Farnham.
From initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care, the PCN Advanced Paramedic Practitioner will demonstrate safe clinical decision-making and expert care for patients within the service.
The PCN APP is expected to attend to patients within the defined list of conditions, including assessment, diagnosis, and treatment to deliver quality patient services in conjunction with the daily duty doctor.
The PCN APP will work collaboratively with the multi-disciplinary team to meet the needs of patients, supporting the delivery as per policy and procedures. There is an expectation that training across the service is maintained to an expected level with the use of appropriate equipment and facilities as directed by the post holder, who is an autonomous allied healthcare professional and will be part of PCN support to practices while working at FICS.
