The Unplanned Care Team is a multi-professional team providing responsive, holistic patient assessments and care, designed to avoid acute hospital admissions and support patients in their usual place of residence 365 days a year and in addition, facilitating discharges from the acute for urgent therapy assessments at home.
The service covers 4 pathways - Rapid response (POA), Discharge to Assess (Therapy facilitated discharge), Unscheduled Planned Care and The Early Intervention Vehicle (EIV - crisis management and assessment). You may be required to see patients on all 4 pathways. Interventions will typically last up to 5 days.
The successful candidate will be a registered Nurse or Paramedic with advanced clinical skills, significant knowledge and experience in the clinical care of adults and older people in a range of healthcare settings.
As a senior member of the team, you will be working in collaboration with other band 7s and will be responsible for leading and managing the unplanned care team and ensuring seamless delivery of care to a diverse population of patients with changing needs.
You will be self-motivated with strong organisational skills and able to make complex clinical decisions autonomously as required. As a senior clinician, you will use your advanced clinical expertise, levels of judgement, discretion and decision making to support patients at home, improve patient outcomes and reduce the likelihood of unnecessary admissions to hospital. You will be required to complete holistic, advanced assessments, giving appropriate interventions as required and have knowledge of community services to plan the ongoing care of patients on the pathway.
Possession of a valid UK driving license and use of a car for work purposes is essential.
