Advanced Clinical Specialist Physiotherapist - Community Stroke

Job summary

An exciting opportunity has arisen in the CCICP Community Stroke team for 22.5 hours per week Band 7 Advanced Clinical Specialist Physiotherapist.

We are looking for a highly skilled and motivated physiotherapist with experience in Stroke to join our dynamic and innovative team. The role requires the postholder to act as an advanced clinical specialist responsible for the assessment, treatment and overall management of patients admitted to the Specialist Community Stroke Rehabilitation Service providing advanced level assessment and intervention for patients with a diagnosis of Stroke.

The postholder will be required to work as part of the wider multi-disciplinary team to support integrated working for this patient group and make relevant onward recommendations as necessary. There will be a requirement to act as lead clinician within this specialist area supporting the operational delivery, planning, co-ordination, and evaluation of the Physiotherapy service provided for patients with a diagnosis of Stroke throughout the patient pathway including service development initiatives.

The team consists of Physios, OTs, SLTs, nursing, psychology services and rehab assistants and works in partnership with the Stroke Association locally to provide a holistic service to the patient.

The service covers areas around South Cheshire including Nantwich, Sandbach, Crewe, Northwich and Alsager. It is commutable from Manchester, Liverpool, Chester and North Staffordshire.

Main duties of the job

1. To act as an advanced clinical specialist responsible for the assessment, treatment and overall management of patients admitted to the Specialist Community Stroke Rehabilitation Service providing advanced level assessment and intervention for patients with a diagnosis of Stroke.
2. To work as part of the wider multi-disciplinary team to support integrated working for this patient group and make relevant onward recommendations as necessary.
3. To act as lead clinician within this specialist area supporting the operational delivery, planning, co-ordination, and evaluation of the Physiotherapy service provided for patients with a diagnosis of Stroke throughout the patient pathway including service development initiatives.
4. To act as an expert source of knowledge, offering specialist clinical advice and support to other healthcare professionals and agencies across organisational boundaries.
5. To work autonomously to manage a defined highly complex patient caseload whilst working as part of a multidisciplinary team.
6. To contribute to the delivery of the service in a variety of defined clinical settings, which may include in-patient wards, community clinics and the patient's home.
7. Travel is an essential requirement of the role.

Job responsibilities

1. To develop and maintain good working relationships with other healthcare professions, social care and other appropriate agencies, to ensure integrated and high-quality patient outcomes.
2. To work as a member of the multidisciplinary team, contributing to decisions with regard to patient care programmes through attendance at ward rounds, case conferences and other multidisciplinary meetings.

11. To refer for diagnostics, as appropriate to the role.
12. To manage a highly specialist caseload, demonstrating appropriate time management and organisation skills.
13. To ensure effective day-to-day organisation and provision of service, including the coordination of junior staff and assistants to meet service priorities.
14. To provide clinical leadership and support to junior staff and assistants, through case supervision, and the development and monitoring of competencies.
