Woodbridge Hill Surgery is looking for an experienced and motivated Advanced Care Practitioner (ACP) to join our friendly, high-achieving, and well-supported team. Main duties of the job

Your Role: As a primary care ACP, you'll play a key role in managing minor illnesses, common GP surgery presentations, and long-term conditions. You'll work both independently and collaboratively, ensuring our patients receive the highest standard of care.

Key Responsibilities:

• Assess, diagnose, and develop effective clinical management plans.
• Provide face-to-face or telephone consultations for undifferentiated patient presentations.
• Manage acute and chronic conditions with confidence and compassion.
• Offer patient education and support, promoting self-management and healthy choices.
• Contribute to the delivery of practice policies and procedures.

Person Specification

Experience

• Experience of working in primary care (not necessarily as an ACP).

Qualifications

• Registered ACP with relevant qualifications (MSc or equivalent in Advanced Clinical Practice)

Skills

• Ability to assess, diagnose, and manage a range of conditions autonomously
• Strong communication skills and a collaborative approach to patient care.
