Job description

Uppingham Surgery is looking to recruit an Advanced Clinical Practitioner with excellent clinical skills and warmth, with a commitment to positively contribute to our provision of high-quality patient centred care.

Uppingham Surgery is a diverse multidisciplinary team looking after 12,800 patients in the beautiful market town of Uppingham, situated just off the A47.

The role offers you the opportunity to work as an autonomous practitioner, working independently to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions.

We are looking for a 6 to 8 session practitioner working between Monday and Friday. The position offers a very competitive salary, generous annual and study leave, with access to the NHS pension scheme.

Main duties of the job

  • Registered nurse or equivalent and have obtained a clinical examination qualification, ideally to masters standard
  • Experience in a similar role or an interest in transferring to a Primary Care setting
  • The ability to utilise your clinical knowledge and experience to provide competent care to meet the health and social care needs of patients
  • Positive and proactive approach to work, with the ability to work under pressure
  • Experience of working in a team and using your own initiative

What we expect from you

  • Provide our patients treatment which is based on evidence and best practice
  • Develop innovative concepts, models, methods and practices which deliver new and improved primary care services to meet the changing needs in Primary Care
  • Provide education and training to other staff and students as appropriate and to take a lead role in encouraging teamwork and sharing best practice
  • An interest or skill in an area of long term condition management
  • Collaborative working with GPs, ACPs, Clinical Pharmacists, Nursing team and non-clinical staff to make positive contributions to the organisation and the continued development of a One Team approach

Job responsibilities

The post holder is an experienced clinical practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements or equivalent for advanced practice must be met.
