An established healthcare provider is seeking Advanced Clinical Practitioners who excel in assessing and treating patients with undifferentiated conditions. This role offers a unique opportunity to work in a dynamic environment, providing urgent care during out-of-hours. With flexible working hours, practitioners can choose between home visits or working in primary care centers. Join a dedicated team that values high-quality patient care and supports your professional growth. If you are passionate about making a difference in patients' lives and thrive in a collaborative setting, this position is perfect for you.
Advanced Clinical Practitioners Needed (ACP / ANP) Are you working at an advanced level assessing and treating all ages of patients with undifferentiated conditions in General Practice? Would you like some flexibility in your work life? Have you thought of expanding your experience in general practice? If you answered yes to these then you might enjoy the variety of work that GP OOH offers.
In GP OOH we assess and treat patients who require urgent unscheduled interventions suitable for treatment by general practice clinicians when the GP surgeries are closed. Our referrals come from either 111, via a dedicated health care professional line used by paramedics and district nursing teams or from the AED or MIU. We currently offer a telephone consultation first system to offer high quality remote patient management via telephone and video technology where appropriate, but if a patient needs a face to face review or examination then this is offered at either of our bases at George Eliot Hospital Nuneaton, Warwick General Hospital, Rugby St Cross Hospital, or Coventry Walk In Centre, as well as via home visits where appropriate.
As a ANP / ACP working in OOH, you'll be treating patients face-to-face or via Telephone Triage with the flexibility to choose whether you'd like to do this in one of our Primary Care Centres as a base shift, or at the patient's own home as part of our home visiting shifts. The OOH service is open from 6.30pm to 8am Monday to Friday, and 24 hours over the weekends, with various shifts on offer between these times. You will also have the flexibility to choose when you would like to work in order to fit in with your lifestyle.
We provide this high quality care via a skill mix of GPs and Advanced Nurse / Clinical and Paramedic Practitioners supported by an experienced operational team of shift Co-ordinators, Assistants, receptionists and drivers. We have access to basic daytime GP notes via GP connect, summary care record, special notes and child protection information sharing. Both the lead nurse and medical lead work in a clinical capacity as well as a managerial capacity and are approachable and supportive. You will also be using our electronic prescribing system. We are looking to add to our existing Self Employed workforce but can also offer Salaried and Bank employment options.
