An exciting opportunity has arisen for an Advanced Care Practitioner/Physio/Paramedic/ANP/Nurse Practitioner to join us at Chelston Hall Surgery in Torquay.
We are looking for an enthusiastic, dedicated and highly motivated individual to join our forward-thinking, dynamic team at Chelston Hall to enhance our team and help us further develop our philosophy of integrated multi-disciplinary working. We are a friendly practice serving the local community. Our list size is approximately 18,500 patients. Our clinical team already comprises of doctors, nurse practitioners, practice nurses, health care assistants, phlebotomists and a pharmacy team.
You will be working over our two sites at Chelston and Barton. We are interested in an experienced individual with advanced clinical skills and a prescribing qualification. General practice experience would be beneficial. You will be expected to help provide the urgent on-the-day team service for our patients. This involves minor ailments but an understanding of long-term conditions would be beneficial.
This may be by face-to-face consultation, telephone consultation or home visit. Senior clinician support for this role is available whenever needed but it is expected that you will have the skills to work independently.
To act as an autonomous practitioner who is able to work independently and in conjunction with other members of the clinical team. The post holder will be an experienced, caring person who can demonstrate highly developed clinical skills and be able to demonstrate critical thinking whilst working within their professional boundaries.
You will be able to demonstrate safe, competent clinical decision-making and deliver expert care, including history taking, assessment, diagnostic skills, treatment and evaluation. You will provide an alternative and complementary source of primary health care alongside services provided by General Practitioners.
You will work collaboratively with the multidisciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. In order to work at this level, relevant professional requirements for advanced practice must be met.
