The ICB currently has a vacancy for a Local Resolution Lead within the All Age Continuing Care (AACC) team.
To manage and develop services in relation to the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. Ensuring high quality, patient focused services to meet the complex needs of those eligible in HWE in line with strategic goals and national policy.
The post holder is responsible for ensuring the safe and effective management of the NHS Continuing Healthcare Service across the local place within HWE and the effective and efficient management of all resources of the service.
The post holder will be the Adults lead for the locality, focusing on adults entering CHC and this pathway. To support the Assistant Director in delivering policies related to Continuing Healthcare and Funded Nursing Care, so that the policies are implemented to a high standard which enable consistent decisions to be made on an individual's eligibility for NHS funded healthcare and then subsequently safe and appropriate clinical care is commissioned monitored and reviewed.
The post is responsible for providing professional expertise to the outcome of the business processes for the Department or Directorate. To have clinical management responsibility for the Continuing Healthcare (Adults with a Physical Disability and Learning Disabilities) and Funded Nursing Care services to ensure that a high quality patient focused and responsive service is provided and monitored within clinical and non-clinical legal governance frameworks (NHS and Private providers).
