Job Title: Adult Social Worker - Front Door Team - Stockport
Location: Stockport
Pay Rate: £30/per hour

A temporary position for an experienced Social Worker is available for a 3-month contract. The role involves working with the Front Door Team and the Urgent Community Response Team. Key responsibilities include:

1. Collaborating with Health colleagues to minimize hospital visits
2. Conducting visits as needed (including same-day responses)
3. Completing Care Act and short-term assessments to promote independence at home
4. Providing Social Work expertise within a multi-disciplinary team

The position is office-based and requires a clean driving license and transport.

Qualifications and Experience:
To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
