Job description

Experienced Social Worker +2 years
The role entails being in the office 4 days a week and 1 day working from home

Lewisham and Greenwich NHS Trust – LGT is commissioned to provide a community rapid response service to Lewisham residents seven days a week (Monday to Sunday from 8am to 8pm, including bank holidays). This service treats patients at home to avoid a hospital admission or readmission. If needed, a short-term intervention can be put in place for the patient for up to five days.

Under this agreement, The Service Provider shall provide services in accordance with this Service Specification.

To provide social work support to the UCR team, the Service Provider will:

  1. Assessments and Care Planning: Conduct timely and comprehensive social care assessments for referred patients, identifying their needs and coordinating care plans that align with their health requirements and social circumstances.
  2. Crisis Intervention: Offer immediate social work intervention to stabilise and support patients in crisis, aiming to prevent unnecessary hospital admissions.
  3. Social Care: Urgent social care interventions and support are integral to enabling people to stay at home or in their usual place of residence, thereby preventing hospital admissions.
  4. Reablement Services: Collaborate closely with reablement services, ensuring that the two-hour UCR services work in tandem with social care emergency crisis support.
  5. Carer Breakdown Response: Provide urgent support in cases where an unpaid carer’s breakdown could lead to a healthcare crisis for the person they care for.
  6. Urgent Referrals: Make an urgent referral to the relevant services working closely with the UCR team.
  7. Collaboration: Work jointly with local authorities, care providers, or other relevant services to ensure comprehensive care and support.
  8. Multi-disciplinary Collaboration: Work closely with healthcare professionals within the UCR team, including nurses, occupational therapists, and doctors.
  9. Safeguarding: Identify and address any safeguarding concerns, ensuring that vulnerable adults are protected.
  10. Coordination of Services: Liaise with other social care services, community resources, and voluntary organisations to arrange and coordinate additional support.
  11. Patient and Family Support: Provide emotional and practical support to patients and their families.

Service Standards and Levels:

  1. National Quality Standards: The social worker must adhere to the standards set by their professional body.
  2. Qualifications: The social worker must hold a degree in social work and be registered with a professional body.
  3. Performance Targets:
    Response Time: Initial contact with referred patients should occur within 24 hours of receiving a referral.
    Assessment Completion: Comprehensive assessments should be completed within 48 hours of the initial contact.
    Safeguarding Response: Any safeguarding concerns must be addressed within 24 hours of identification.
  4. Audit and Monitoring: Regular audits should be conducted to ensure compliance with service standards.

Data Requirements: London Borough of Lewisham will provide a quarterly report on the KPIs outlined in the service objectives.

LGT Responsibilities:

  1. Referral Management: Provide accurate and timely referrals.
  2. Collaboration and Support: Support the integration of the social worker into the UCR team.
  3. Data Sharing: Ensure secure and efficient data sharing protocols are in place.
  4. Training and Development: Offer opportunities for the social worker to participate in relevant training provided by LGT.
  5. Complaints and Incident Management: Actively engage in the agreed complaints and serious incidents reporting procedures.
