Job description

Job Summary: Under the supervision of the Team/Program Manager, the field clinician is accountable for effectively assessing patients for eligibility for homecare services and admitting eligible patients for homecare services following the admission process. Position may also include revisits, resumptions of care, recerts, and discharges as needed to ensure safe and effective patient care. This position coordinates the interdisciplinary care, monitors and adjusts treatment to achieve positive therapeutic outcomes, and identifies opportunities for process improvement. These responsibilities will be carried out in accordance with organization policies/procedure and standards consistent with Federal, State and accreditation regulations.

Specifications, Education: Equivalent education required for RI licensure required, Bachelor’s Degree (min) in relevant discipline strongly preferred.
