The Admission / Liaison RN Hospice plays a crucial role in evaluating patient referrals for hospice care, coordinating pre-admission activities, and ensuring the continuity of care for patients in various settings such as homes and healthcare facilities. This position involves educating families about hospice services, verifying insurance coverage, and collaborating with medical professionals to facilitate smooth admissions. The RN is also responsible for community outreach and training to promote hospice services and ensure compliance with regulatory standards.
Job Overview
The position is a member of the hospice staff who determines appropriateness of referrals, coordinates and conducts pre-admission activities, completes admission when determined hospice appropriate, monitors inpatient care at contracted hospitals, performs in-service training and community education activities. This position performs duties in a variety of settings including patient’s home, acute care setting, skilled nursing facility or assisted living.
