Job description

An exciting opportunity has arisen for a fixed term (12 months) post as a Health Care Support Worker (HCSW) to join an established and welcoming Community Mental Health Team (CMHT) in Annan.


The post holder will have a key role in providing support to patients in line with patient centred care plans, risk assessments and monitoring physical health care for specific moderate-severe mental health conditions. Physical health monitoring would include Venepuncture, ECG, Clozapine and Lithium monitoring. Relevant training will be provided to the successful candidate.


The post holder will support the nursing team in the delivery of care and be part of a larger multi-disciplinary team focused on joint working to deliver safe high quality care. The post holder will provide general health advice, education and support patients to achieve recovery goals. The focus will be on promoting self-care and recovery whilst enabling people with mental health problems to be as independent as possible.


Team priorities include collaborative working, patient enablement, care assurance and strength based approach for patients. The successful candidate will be based within the CMHT base at Annan Health Centre and will be expected to travel within the area. The post holder will work in partnership with service users, families, carers and other agencies across the Annandale & Eskdale locality.
