Supportive Housing Case Manager

Belleville and Quinte West Community Health Centre (BQWCHC)

Temporary - 18-month full-time contract ($52,980.20 - $64,428.00 /year based on 35 hours/week)

Job Summary

Belleville & Quinte West Community Health Centre (BQWCHC) offers community-focused health promotion, illness prevention, and primary health care services. The Centre supports diverse populations of all ages and stages of the life cycle, with an emphasis on those who are high risk and/or are experiencing barriers to accessing services.

The Supportive Housing Case Manager provides direct outreach support to BQWCHC sponsored tenants of the Hastings County funded “Home for Good” (HFG) building in Belleville, Ontario, as well as to the BQWCHC sponsored tenants residing at Great Saint James and Sidney Place in Belleville, Ontario as part of the Quinte Community and Housing Partnership. The Supportive Housing Case Manager builds and maintains supportive relationships with the tenants; supports the development of individualized care plans focused on accessing the resources needed to successfully maintain housing and the development of life skills and self-management skills to promote improved quality of life. The Supportive Housing Case Manager works collaboratively with BQWCHC staff and other community partners.

Accountability

This position is accountable to the BQWCHC Manager of Community Health Programs and Engagement. The Supportive Housing Case Manager is a member of the Community Health Team.

Key Areas of Responsibility

  • Identify and triage clients eligible to acquire tenancy and maintain full occupancy of all units allocated to BQWCHC through the various supportive housing partnerships, including Home For Good (5 units), Great Saint James Place (2 units), and Sidney Place (5 units).
  • Serve as the main point of contact between BQWCHC supportive housing tenants and their care teams, including the Home for Good Coordinator and the Community and Housing First Partnership Coordinator, to ensure comprehensive and coordinated care and support; and to act as an advocate for supportive housing tenants among their care teams and with other related organizations/supports.
  • Provide ongoing case management support, assessing client status by gaining an understanding of socio-economic, psychological outlook, financial and legal needs and facilitate access (referrals) to the available community resources to address those needs and goals.
  • Assist tenants to develop skills of independent living (budgeting, cleaning, daily life skills, etc.).
  • Help tenants to build and maintain positive, supportive, healthy relationships and a sense of community.
  • Assist in the development and implementation of programs/groups to support successful ongoing tenancy for all supportive housing tenants and other qualified participants as appropriate, including instrumental supports and skills for independent living, self-management, relationship building, and social inclusion.

Qualifications

  • Baccalaureate degree in Social Services field (or related field) or a sufficient combination of experience and education.
  • Demonstrated understanding of the social determinants of health and client/person-centered approach.
  • Experience working with individuals facing barriers to access to service and experiencing health inequity.
  • Familiarity with the Housing Services Act, Residential Tenancies Act, local housing challenges, and social services sector a definite asset.
  • Experience developing programs, including the development, implementation, and facilitation of group programming.
  • Creative problem solving, system navigation skills and client advocacy skills.
  • Excellent interpersonal skills (verbal and written).
  • Ability to work effectively and efficiently, both independently and as part of a team.
  • Computer skills and a valid driver’s license and reliable vehicle (travel within Quinte) are required.
