Reports to: Program Manager, Housing Services

Vacancies: One (1) Hourly Rate: Pay Band 6
$31.27 - $36.33

We are seeking a Case Manager who is enthusiastic and passionate about opportunities to work with individuals in the community. Our Case Managers are key members of many frontline teams, including the Transitional Rehabilitation Housing Program. This role contributes to the support of our clients in providing invaluable services to individuals with addiction and/or mental health concerns within the Kingston, Frontenac, Lennox and Addington (KFLA) communities.

Job Summary:
The Transitional Rehabilitation Housing Program Case Manager, reporting to the Program Manager, works as part of an interdisciplinary team providing psychosocial rehabilitation and person-centered support to individuals within the Transitional Rehabilitation Housing Program. The Case Manager works independently and in conjunction with partners, carrying out treatment rehabilitation and support functions to facilitate successful community living for Providence Care Forensic Mental Health clients under the jurisdiction of the Ontario Review Board (ORB), using a recovery oriented model of care. This position aims to build the capacity and strengths of individuals and works as an integral member in building a sense of community within the residential setting. Responsibilities include participating in comprehensive assessments of clients, coordinating individualized treatment plans, and providing rehabilitation support, including assistance with instrumental activities of daily living (IADL) and in-home support and supervision.

Minimum Qualifications:
University Degree in a health-related discipline. Three years relevant experience providing services to individuals living with serious mental illness, preferably in a community setting. Certification in First Aid and CPR an asset. Applied Suicide Intervention Skills Training (ASIST) an asset. Experience working within a case management model preferred. French language proficiency considered an asset.

Knowledge, Skills and Abilities:
Knowledge of and ability to apply discipline-specific principles and practices to individuals living with serious mental illness, within a client-centered approach. Demonstrated in-depth knowledge of mental health symptoms, issues, psychotropic medications, and associated side effects. Knowledge of issues related to addictions is an asset. Ability to incorporate best practices in community support. Demonstrated assessment and crisis intervention skills. Advanced clinical reasoning and decision-making skills. Ability to effectively utilize community resources and supports. Ability to work under pressure. Knowledge of relevant legislation, including the Health Care Consent Act and Mental Health Act. Excellent communication and interpersonal skills. Well-developed problem-solving, prioritization, and conflict resolution skills. Ability to work autonomously and collaboratively in a multidisciplinary team environment. Basic proficiency in computer skills; MS Office preferred. Valid Ontario Driver’s License and access to a reliable vehicle. Required to provide a satisfactory criminal reference check (CRC) with Vulnerable Sector Screening prior to hire.
