What we are looking for:

We are seeking a passionate, creative and innovative System Planner & Data Analyst who will excel at providing analytical support for the Eastern York Region North Durham (EYRND) Ontario Health Team (OHT) and supports the development of population health initiatives aimed at system integration and improvement.

Reporting to the Manager, Planning & Integration, and working directly with other members of the Decision Support and EYRND OHT, you will:

Analytical & Critical Thinking:

  • Analyze and interpret complex health system information and make recommendations for system improvement
  • Support quality improvement processes for new integrated care models
  • Develop KPIs, data visualizations, and reports to enable and support decision-making processes for OHT and its stakeholders;
  • Perform population health analysis via OHT Dashboard, Hospital, Intellihealth Ontario, and CIHI Databases to support OHT initiatives

Communication & Reporting:

  • Prepare and submit grant/funding applications that advance integrated collaborative care, coordinate reporting to the Ministry of Health (MOH) or other groups, and support the evaluation of integrated care initiatives
  • Initiate knowledge translation activities across the province to share information, best practices and learnings (e.g. with other Ontario Health Teams)
  • Prepare briefing notes, presentations and other materials for senior audiences
  • Lead and facilitate stakeholder engagement via Miro and other engagement tools
  • Support reporting to various external stakeholders including Ontario Health, and the Ministry of Health and Long-term Care
  • Facilitate the uptake of knowledge by local decision-makers across OHT through the presentation of findings, education sessions, and one-on-one interactions as required
  • Analyze complex datasets and run SQL scripts to extract, interpret, and summarize data insights, supporting data-driven decision-making and enhancing operational efficiencies of OHT programs

Project Management:

  • Lead, organize and facilitate project-related meetings, develop project reports, presentations, statistics and other related documents, as required
  • Continuously evaluate and assess each project milestone closely to identify lessons learned and best practices for implementation in future phases

What you bring to the role:

  • Masters Degree in Public Health, Business or Healthcare Administration or equivalent experience required
  • Three (3) or more years of experience in a decision support environment in healthcare including experience preparing analyses of operational, and quality measures
  • Thorough understanding of approaches to developing strong stakeholder buy-in and change management
  • Thorough understanding of project management methodologies, practices and tools
  • Demonstrated clinical and stakeholder engagement expertise
  • Strong leadership abilities
  • Ability to work effectively in both a team setting and independently
  • Superior interpersonal, communications skills and ability to successfully build relationships
  • Demonstrated effective analytical and problem solving skills
  • Ability to discover, analyze and interpret healthcare and population data/information, ability to draw inferences, create data visualizations and recommend next steps; Skills in managing and manipulating large databases that include personal health information and conducting data linkage
  • Ability to write complex SQL queries, including joins, subqueries, aggregations, and window functions
  • Experience applying various statistical or data science techniques such as: Forecasting, Predictive Modeling, Correlation and Regression
  • Secondary research skills (using academic and grey literature sources)
  • Knowledge of quality improvement and evaluation methods
  • Proactive and customer-focused work approach
  • Proactively engage stakeholders and subject matter experts to uncover links and opportunities in relevant data to support decision making and forecasting
  • Strong understanding and analysis of equity issues from an anti-racism, anti-oppression, resisting anti-Black racism lens
  • Strong writing and presentation skills with the ability to translate complex ideas and concepts for both executive and public audiences
  • Proven ability to work in a fast paced environment; adaptable, flexible, positive and responsive to change
  • Ability to think creatively
  • Excellent time management and work prioritization skills

Work Schedule:

Monday to Friday, full-time. Hybrid. Please note schedules may change due to operational needs.

Compensation:

BAND E

Range: $47.57 - $62.56
