Reporting to the Professional Practice Leader and under the direction of the Patient Care Manager, the Social Worker, Older Adult and Dementia Caregiver Resilience Clinic provides advanced therapeutic services for older adults and their caregivers in the context of dementia.

Working from a systems and structural theory-based model, the Social Worker supports a designated caseload and plays a vital role in care coordination, education, and community engagement.

Key Responsibilities:

• Provide advanced clinical services, including complex assessments, individual and group therapy, and caregiver education.
• Conduct comprehensive biopsychosocial assessments and maintain accurate patient/resident records.
• Deliver therapeutic counselling to patients, families, and caregivers.
• Support and lead the interdisciplinary team in discharge planning.
• Offer consultative services to staff and community health professionals.
• Coordinate and facilitate clinical/case conferences.
• Develop and deliver in-services and educational presentations for staff.
• Participate in local committees and team meetings.
• Liaise with community agencies, hospitals, mental health services, and charitable organizations.
• Promote inclusive care practices that respect cultural, religious, and ethnic diversity.
• Stay current with developments and best practices in the Social Work profession.

What You Bring

Education, Training, and Experience

• Master’s Degree in Social Work.
• Completion of post-master’s training in evidence-based therapeutic modalities, such as:
• Mindfulness-Based Therapy
• Cognitive Behavioural Therapy (CBT)
• Or other relevant, equivalent modalities.
• Minimum of two (2) years’ experience working with individuals living with dementia and supporting their caregivers.
• An equivalent combination of education, training, and experience may be considered.
• Current full registration with the British Columbia College of Social Workers. (Clinical Registration is preferred)

Skills and Abilities

• Strong knowledge of dementia care, including pathophysiology, neuropsychiatric symptoms, and therapeutic interventions such as Mindfulness-Based Dementia Care and CBT.
• Skilled in psychotherapeutic and psychoeducational group facilitation, with the ability to communicate effectively with older adults experiencing cognitive impairment.
• Proven ability to work independently and within inter-professional teams, with sound understanding of community resources and referral processes.
• Committed to practicing cultural safety and humility, with the ability to educate and consult with clients, families, and healthcare professionals.
• Able to manage a complex workload, with knowledge of healthcare regulations and proficiency in using computers and related software.

Your Day to Day

• Participate in the development and implementation of treatment practices, guidelines and protocols within the team to promote excellence in social work practice within the Older Adult and Dementia Caregiver Resilience Clinic.
• Participates in complex care planning with community agencies and assists in coordinating treatment follow-up with other team members and community partners.
• Establishes and maintains effective collaborative and constructive liaison relationships with various social workers, academic institutions, hospitals and other agencies.
• Provides advanced therapeutic clinical counseling services to clients, families and groups in designated program area.
• Conducts comprehensive social work psychosocial assessments in designated program area.
• Formulates and implements care and treatment plans.
• Maintains client records through electronic methods.
• Participates in program planning and development of clinical practice guidelines.
• Takes initiative in keeping up-to-date on research and best practices.
• Maintains knowledge of community resources.
• Initiates and participates in psychosocial research.
• Supports patients/Families/Teams in understanding legislative processes.
• Advocates for the patient, family or caregiver to address gaps in resources.
• Provides practicum supervision and teaching to master’s level Social Work students.
• Participates in relevant committee work.
• Performs administrative tasks.
• Performs other related duties as assigned.
