Job Summary:

Utilizing clinical knowledge and skills, Social Workers develop collaborative relationships with aging patients and their families to promote patient-centered goals. In this role, the Social Worker will facilitate psychosocial assessments of the emotional, physical, cognitive, and psychosocial needs of patients and their families. As an integrated member of the interprofessional team, the Social Worker will provide discharge planning, referrals and resource navigation, advanced care planning and counseling to patients and their families, with a focus on ensuring successful transitions in care. The Social Worker will facilitate referrals to Home and Community Care services, along with community resource navigation as required. The Social Worker in this position demonstrates a high degree of practice excellence, and functions as a clinical resource for the interprofessional team as well as the patient and family. This position reports directly to the Manager and Interprofessional Practice Chief for Social Work.

Responsibilities include but are not limited to:

  1. Provides psychosocial assessments, crisis intervention, discharge planning and counseling to patients and their families.
  2. Provides support/consultation to other disciplines on the team re: psychosocial issues particularly related to complex patient/family situations.
  3. Acts as the team expert on discharge planning and patient flow.
  4. Acts as a team liaison with Ontario Health at Home.
  5. Acts as a linkage and communication conduit between the family and the care team.
  6. Establishes community linkages and outreach with relevant agencies and other Baycrest programs and services.
  7. Participates in teaching and other student supervision activities for health professional students assigned to this unit.

Qualifications include but are not limited to:

  1. Masters of Social Work degree.
  2. Current and valid registration with the Ontario College of Social Workers and Social Service Workers (‘OCSWSSW’).
  3. Experience working in a healthcare setting preferred.
  4. Excellent interpersonal, communication and problem-solving skills.
  5. Basic computer skills – Meditech, Microsoft Word and Excel, RM&R preferred.
