As a member of the Substance Use & Concurrent Disorders team, you will use best practices to provide person-centred Social Work services to clients with substance use disorders and co-occurring mental health and physical health problems and their families. This includes assessment, crisis management, therapeutic intervention, group therapy, and discharge planning, in collaboration with the clinical team, client, families, and hospital and community partners. This position specifically requires expertise in harm reduction interventions, evidence-informed psychological treatment approaches (i.e., motivational interviewing, cognitive behavioural therapy, dialectical behaviour therapy), and care planning with community and hospital partners.
