Cancer Care Alberta (CCA) is seeking a compassionate and dedicated Social Worker II (SWII) to join our Supportive Care Services team. Our team of healthcare professionals is committed to enhancing the quality of life for those affected by cancer, providing support from diagnosis through treatment and beyond. CCA professionals are leaders in cancer prevention, diagnosis, treatment, survivorship, and palliative care, all grounded in world-class research. As a Social Worker II, you will play a vital role in empowering patients experiencing cancer to address the social determinants of health.

Key Responsibilities:

  • Referral triage, screening/intake, resource counseling, supportive counseling.
  • Working collaboratively with a team of healthcare professionals to enhance the patient experience.
  • Developing and facilitating psychoeducational groups.
  • Participating in quality improvement initiatives, research, and service evaluation.
  • Engaging in continuous professional development and education.
  • Supporting orientation and mentorship of new employees and students.
  • Contributing to a collaborative team environment with excellent time management and communication skills.
  • Connecting patients to non-AHS programs such as Wellspring Alberta, the Canadian Cancer Society, and other community programs.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provide a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation. A minimum of 1 year of post-degree (Social Work) experience. Demonstrated experience working in an urban adult cancer care setting, palliative care, and outpatient care. In-depth knowledge of advance care planning, income security programs, housing, and support services. Experience providing support in the context of grief and loss. Experience providing supportive counseling. Demonstrated skills in assessment, intervention, system navigation, and case management. Demonstrated dedication to professional development. Proficient with Microsoft Suite (e.g., Excel, PowerPoint, Word, Outlook).

Preferred Qualifications:

Experience working within a chronic disease management framework with adult populations in an urban outpatient setting within the last 2 years. Connect Care (Epic) training completed for Social Workers in AHS. Experience working in an adult acute care setting within the last 2 years. Experience providing virtual care (phone, Zoom, Telehealth) in the last two years. Experience developing partnerships/collaborations within and across organizations. Experience facilitating groups.
