Your Opportunity:

Social Workers provide services to the residents/patients within the various programs of St. Michael’s Health Centre, including Post-Acute Rehabilitation, Palliative Care and Long-Term Care programs. Social Workers work with internal and external stakeholders/partners. The Social Worker II is part of the greater interdisciplinary team that includes RNs, LPNs, HCAs, Occupational Therapists, Physical Therapists, Recreation Therapists, Therapy Assistants, Registered Dietitians, Pharmacists, Speech Language Pathologists, Unit Clerks, and Physicians.

Description:

The Social Worker II (SW) provides services to clients and families including psychosocial assessment, crisis intervention, education, treatment, advocacy, resource finding, consultation, and discharge planning. The SW II is responsible for the provision of services in accordance with established standards, and the Mission, Vision, Values, policies and procedures of Covenant Health. This position is covered by the terms and conditions of the HSAA Paramedical Professional/Technical Collective Agreement.

Required Qualifications:

Graduate of a Social Work program or recognized equivalent required. Active registration with the Alberta College of Social Workers (ACSW) required.

Additional Required Qualifications:

Minimum 1 year of related experience required. Understanding of community integration and resources. Knowledge of relevant legislation. Knowledge of challenges in providing supports with limited resources. Position may include significant periods of time sitting while interviewing patients/families, computer work etc. Ability to perform multiple tasks in a fast-paced environment. Ability to participate in sensitive and challenging patient situations and diverse situations. Excellent verbal and written communication skills.

Preferred Qualifications:

Geriatric or Long-Term Care experience. Connect Care experience.
