Your opportunity is in a new 19 bed Transition Care Unit at the QEII Ambulatory Care Centre to support patients as they transition from acute care to another level of care or to return home. You will be part of a multi-disciplinary team with a strong focus on collaboration both within the larger care team and other community providers as appropriate to provide the best care during stay and upon discharge. Reporting to the Area Manager Allied Health, the Social Work II will provide a range of Social Work services to patients and their families on the unit. These services are primarily clinical in nature and require expertise in financial and resource management, psychosocial assessments, counseling, case management and discharge planning. This also involves consultation and collaboration with medical staff and community agencies. This position requires the ability to work independently with occasional review, consultation and evaluation from a Manager and the ability to work as part of an inter-disciplinary team.
In addition to excellent wages and benefits, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Social Worker II, you will be a member of an inter-disciplinary team and provide a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:
Completion of a recognized Bachelor of Social Work degree required. Registration with the Alberta College of Social Workers (ACSW) required. Current BCLS-HCP certification required. Excellent communication skills, written and verbal. Ability to establish positive working relationships with a wide variety of patients and families. Ability to organize and set priorities. Considerable initiative and judgment required. Experience working on an inter-disciplinary team required.
Preferred Qualifications:
Minimum of two years’ experience in clinical area of medical social work recommended, and specific experience in the acute care/continuing care environment is preferred. Experience with electronic charting is required, Connect Care training is preferred.
