About The Position

The Home Health Transformation initiative is designed to support the growing demand for home-based care, driven by population growth and the increasing preference from clients to receive care at home. It aims to ensure that people can live independently with the support they need, in their own homes and communities. By incorporating digital health tools, telehealth, and other technologies, the initiative aims to improve communication and coordination among healthcare teams, as well as provide clients with easier access to care and support while in their homes.

The Social Worker will have a key role in a home health transformation team working both independently and collaboratively to provide exceptional social work services. Your responsibilities will include providing direct psychosocial care, counseling and group work, system navigation and advocacy, education, and linkage to relevant specialized complex care resources while facilitating client self-management. The Social Worker is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.
