About the job:
Social Workers perform comprehensive assessments, care planning, and treatment coordination for clients and their families. Collaborating with a multidisciplinary team, you'll provide psychosocial care, counseling, system navigation, and advocacy, while connecting clients to specialized resources and supporting their self-management. Your role focuses on improving the health and social well-being of the identified population.

Qualifications:
Bachelor’s or Master’s Degree in Social Work from an accredited university
Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training, and experience
Current full registration with the British Columbia College of Social Workers
Current valid BC driver’s license.
