Job Summary

The Mental Health Social Worker is responsible and accountable for providing shockingly excellent patient/customer care as a member of the Mental Health Team.

Primary Responsibilities

• Function as a member of a multi-disciplinary team which provides:
• Assessment, treatment planning and short-term counselling of clients with serious mental illness in the Adult Outpatient Mental Health program.
• Responsibilities may include psycho-social work with individuals and in group therapy.
• Documentation of clinical assessments, treatment plans and client clinical progress.
• Ability to independently lead client groups, understanding group dynamics and have excellent facilitation skills.
• Ability to perform and report to the team on client intake assessments.
• Liaison with, and promotion of, good working relationships with team members, the hospital community and community agencies.
• Provide education to the community/hospital.
• Collection of statistical data.
• Committee membership as appropriate.
• Perform other duties consistent with the job classification, as required.

Qualifications

• Masters in Social Work (MSW).
• Three to five years experience working with seriously mentally ill clients.
• Ontario College of Social Workers and Social Service Workers (OCSWSSW).
• Advanced counselling and assessment skills with various short-term counselling modalities, including Brief Solution Focused model, cognitive behavioural and dialectical behavioural therapy.
• Demonstrated ability to work collaboratively in an interprofessional team environment as well as independently.
• Regular attendance is a requirement of the position.
• Role models both professional and the organizational core competencies.
• Nonviolent Crisis Intervention Course (CPI) or equivalent.
• Ability to de-escalate a crisis situation.
• Good working knowledge of Social Assistance forms.
• Understanding of the Mental Health Act.
• Understanding of Mental Health Disorders.
• Good working knowledge of Social Assistance programs/forms.
• Demonstrated excellence in communication, both written and verbal.
• Demonstrated analytical and problem assessment skills.
