Job Description
In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority, (Island Health) patient safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Working without general supervision as a member of a multidisciplinary health care team providing patient-centered care, the Social Worker provides assessment, care planning, intervention, consultation, and referral for services to patients and their caregivers/families where the social, economic, emotional or psychological circumstances of the patient presents barriers to care and/or an optimum level of function.
Bachelor of Social Work degree from a recognised university; Current full registration with the BC College of Social Workers; Post-degree preparation in program-specific areas, such as Gerontology or Palliative Care, preferred; Working knowledge and familiarity with various community based health and human services; and two (2) years of recent, related experience; or an equivalent combination of education, training and experience.
