Position Summary
Reporting to the Manager Department of Social Work, you will be responsible to provide quality social work services congruent with the Code of Ethics and Standards of Practice of the Ontario College of Social Workers and Social Service Workers. The registered social worker will work within an interdisciplinary team model of care and implement a full scope of practice in clinical care areas including: psychosocial assessment, family engagement, brief supportive interventions, resource identification, coordination and/or utilization of available resources, education, and advocacy to support the transition of patients from hospital to post-acute or community based locations. Practice will focus on the application of an anti-oppressive, person-centred model of care and recovery orientated and harm reduction approaches for persons with acute and chronic health issues, including cognitive impairment, dementia, mental health issues, concurrent disorders, addictions and caregiver burnout. The Social Worker will work as a member of the interdisciplinary teams of these programs. You will also be required to maintain appropriate case records and departmental statistics as well as perform other related duties as requested/negotiated by or with the Manager.
