Job Overview

The Wellness Nurse is responsible for ensuring the overall well-being of our residents. This role requires a strong nursing background, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Key Responsibilities

  • Identify Ongoing Needs: Collaborate with the Resident Care Director (RCD) to identify on-going needs and services required by residents to promote the highest quality resident services.
  • Ensure Proper Follow-Through: Participate in evaluations and documentation for residents with changes in healthcare services or status.
  • Assist with Resident Evaluations: Conduct resident evaluations and gather data at move-in, 30 days post-move-in, semi-annually, and with any change in status.
  • Conduct Monthly Wellness Visits: Visit residents monthly to monitor their health and wellness.
  • Monitor Falls and Accidents: Monitor and follow up with residents who experience falls, accidents, or incidents; document and report as required and directed.

Qualifications and Skills

  • Nursing Background: Graduate of an approved college/school of nursing and maintain a current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Experience: Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
  • Communication Skills: Possesses written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care.
