Overview

This is a Full-Time Term position for Mat-Leave Coverage.
The purpose of the CHN is to provide community health services in Ktunaxa communities in accordance with Ktunaxa values and culture and to promote the prevention of disease, maintenance of health and the overall health and well-being of the community.
This position delivers a comprehensive health program, which includes: health promotion, disease prevention, health education, community development, immunizations, maternal child health, injury and illness prevention. Assist patients to achieve their optimum level of health in situation of normal health, illness, injury, or through the process of dying.

Promote a safe and healthy environment in homes, schools and throughout the community. Assess, plan, implement, document and evaluate care to ensure a coordinated holistic approach which best meets the needs of the patient (assessments may involve taking x-rays of extremities and/or lungs in order to make appropriate diagnosis).

Act in a manner consistent with their professional code of ethics, responsibilities and standards of practice. Use a holistic approach to facilitate individual learning of patients and their families in relation to patient illness or injury (i.e. self-care, health promotion, etc.).

Responsibilities

  • Maintain client and administrative records following professional and legal guidelines in a confidential, concise and accurate manner.
  • Safeguard the trust of patients that information learned in the context of a professional relationship is shared outside the health care team only with the patient's permission or as legally required.
  • Communicable Disease Control (CDC) Programming.
  • Assure a comprehensive CDC program, including but not limited to a focus on influenza, HIV, Hepatitis A, B & C, Sexually Transmitted Infections (STIs), Tuberculosis (TB) and vaccine preventable diseases.
  • Provide community awareness programming on immunizations and other communicable diseases (prevention, early detection, transmission & control strategies).
  • Plan, implement and evaluate immunization programs for infants, children, adults, elders and members of the community deemed to be at higher risk.
  • Assess needs/strengths/resources, plans, implements and evaluates the TB Programming for the community as per the BCCDC TB Manual (TB Services for Aboriginal Communities (TBSAC).
  • Ensure that required training is completed and competency is maintained (including current immunization certification, transportation of dangerous goods (TDG), and CPR certification).
  • Ensure that the CDC references are kept up-to-date.
  • Ensure the regular maintenance of CDC equipment and that BCCDC recommended CDC supplies is available.
  • Ensure that cold chain principles are adhered to.
  • Ensure that mandatory CDC reporting is completed in a timely fashion.
  • Provide reproductive health, sexual health education to on-reserve schools.
  • Provide training for school staff: Epi Pen, Epilepsy and Naloxone.
  • Assess needs/strengths and resources, plans, implements and evaluates prenatal, postnatal and newborn programming (including linkage to Canadian Prenatal Nutrition Programming).
  • Provide prenatal, postnatal and newborn education and support programming that includes topics such as injury prevention, SIDS, nutrition, lifestyle and health practices, growth and development, prenatal care, newborn care, breast feeding, physical & emotional care, and when to seek medical attention. This programming may be delivered in group settings and/or one-to-one sessions.
  • Assess needs/strengths/resources, plans, implements and evaluates child health programming.
  • Provide assessments and screening aimed at early intervention for identified health and development concerns (vision & hearing screening, speech & language development, physical growth).
  • Provide referrals to other health professionals and programming should a delay or health concern be identified.
  • Provide information and counselling for parents, other caregivers or staff on topics including but not limited to injury prevention, growth and development, parenting, anticipatory guidance, nutrition, oral health, and environmental risks such as sun exposure, wood ticks and water safety.

Qualifications

Bachelor of Science in Nursing Degree
Current registration with the (British Columbia of Nursing Professionals (BCCNP)
BCCDC Immunization Certification
Current CPR certification
Current Level 1 First Aid
3 years of experience working as a public health nurse or community health nurse
In depth knowledge in areas of: prenatal care, infant care and immunizations, diabetes care, chronic disease management.
Ability to adapt clinical and professional skills to the community and cultural setting
Preferred First Nations community experience
Knowledge and skills in community nursing
Knowledge and experience in aspects of community development, social and political systems and community health issues of the Ktunaxa communities
Demonstrated problem solving and critical thinking skills
Strong interpersonal skills and ability to work as an effective member of a health team
Demonstrated effective written and verbal communication skills.
MS Word and Electronic Medical Record experience an asset.
Knowledge of Ktunaxa history, traditions, culture, society and family life
A valid B.C. Driver’s Licence and reliable vehicle
Must undergo a criminal records check RCMP/Ministry of Health
