As a Psychiatric Rehabilitation Facilitator at RHD, you will be working within a program and in the community with the following schedule: Wednesday & Saturday: 3-11pm, Thursday & Friday: 11am-11pm.

Position Summary

The Psychiatric Rehabilitation Facilitator provides psychiatric rehabilitation services, peer support, and tenant support services for individuals with mental health needs to assist in bridging the gap between physical health care and behavioral health treatment. This service may be provided in different settings including community-based support, Program participant’s personal residence, a group home, program day services locations and other environments as needed.

Program Summary

New Foundations is comprised of group homes for adults with mental illness in Bryn Mawr, PA. The program is located in the community and provides supervision, training, and guidance in daily living skills. Programming and service delivery is guided by the principles of recovery to assist individuals to achieve life goals.

Essential Duties and Functions:

Direct Care:

  • Utilize motivational interviewing to effectively identify the member’s strengths, needs, motivation, triggers, and goals in managing life circumstances.
  • Provide skill-building training and support in identified areas of concern, assisting in recovery management, identification of personal strengths/ assets, development of a strength-based recovery plan /recovery goals, and strategies to meet goals.
  • Assist in identifying or re-connecting to natural supports including but not limited to peer recovery supports and self-help groups in the community.
  • Assist with accessing and navigating community and individual support services and programs including but not limited to:
    • Mental/Behavioral health and/or D&A service system.
    • Mental health court.
    • Adult Probation.
    • Justice Related Services and the legal system.
    • Housing services.
    • Assist in scheduling appointments as needed.

Team-based Collaboration:

  • Participate in treatment team meetings, continuing care (aftercare) meetings, as requested by the member.
  • Provide telephonic assistance to members and family members concerning recovery tools and resources.

Administrative:

  • Utilize electronic health record to document and bill for services through electronic record software.
  • Complete all other assigned tasks as designated by the Program Director.

Qualifications:

  • An associate degree and two (2) years’ work experience in mental health direct service. One (1) year of which must be work experience as Psych Rehab Facilitator OR
  • A bachelor’s degree and one (1) year work experience in mental health direct service OR
  • A High School Diploma and current CPRP certification with a minimum of four (4) years work experience in mental health direct service.
  • CPRP certification must be obtained within 2 years from the date of hire.
  • Valid driver’s license.
  • Ability to pass all required background checks.

Working conditions:

Psych Rehab Facilitator may be required to meet with participants and team members in the field to provide service delivery or for quality assurance purposes. Also, this position may work in a long-term residential setting. This may include the need to use public transportation and walking to locations in both warm and cold weather conditions throughout the year.
