The Opportunity: The Primary Care Nurse provides comprehensive primary health care to our patient population, supporting CSFS’s vision of integrated services, working collaboratively with multidisciplinary team members. Within the scope of practice of the Registered Nurses in British Columbia, the Primary Care Nurse will focus on health promotion, disease prevention, rehabilitative care, supportive care, administration, research, data management, and quality assurance. The Primary Care Nurse provides a vital link in the continuity of care between physicians and Nurse Practitioners and Community Health Nurses working with the teams and providing follow-up/ coordination support when practitioners are not in the community. The Primary Nurse also plays a vital role in assisting marginalized populations, including youth, to access care and connect unattached patients to a provider.
- Delivers patient/client care in accordance with established standards, policies, procedures and the goals, objectives, guidelines and directions of Carrier Sekani Family Services.
- Assisting with child development checks, newborn home visits, pregnancy tests and contraception, including administration of injectable contraception.
- Outreach to Sk’ai Zeh Yah youth center, providing care within the scope of practice and connecting youth to other healthcare providers as needed.
- Assisting with patient check-ins and encouraging patients to access care in order to increase access to care and reduce barriers to health care.
- Assessing, planning, implementing, documenting and evaluating the care needs and goals for individual patients/clients and their caregivers/families.
- As part of the Primary Care Team, participates in case conferences, treatment plan reviews, and staff meetings both by distance, using a variety of communication resources such as telephone, the internet, and videoconference and in person.
What You Will Bring to The Role: - Baccalaureate Degree in Nursing and/or Registered Nurse;
- Current practicing registration with the College of Registered Nurses’ of British Columbia (CRNBC)
- Acceptable experience in a clinical health setting or cross-cultural health promotion field;
- Transportation of Dangerous Goods (TDG) course;

Wage Range: The target hiring range for this position is $43.27-$56.03/hour. Candidates will typically be hired, transferred or promoted between the minimum and midpoint of the wage range. Placement within the wage range is dependent on the applicants’ role specific knowledge, skills, abilities, related experience and in consideration of factors such as market conditions and internal equity.
