Job Overview

Sanctum Care Group's Prenatal Outreach Resource Team (PORT) is seeking 1 Case Manager to join our team, which is committed to delivering high quality, individualized holistic care.

The Case Manager will provide support and services through a harm reduction and trauma-informed lens in a community setting. This position provides intensive case management to prenatal and postnatal women who are affected by addictions, mental health, poverty, and are at risk of having their child apprehended at birth.

Responsibilities

  1. Provide coordination of psychosocial components of individual care plans.
  2. Facilitate referrals to appropriate agencies to assist in attaining individual goals.
  3. Provide supports and advocacy for clients with multiple issues affecting their ability to parent.

Qualifications

  1. Ability to communicate and maintain effective working relationships with clients from diverse social, economic, and cultural backgrounds, as well as with team members and health care providers.
  2. Knowledge and awareness of Indigenous culture and history, with emphasis on child welfare and impacts of trauma.
  3. Knowledge of service providers and supports related to this clientele.
  4. Experience working in an outreach setting.
  5. Understanding of issues related to addiction, including harm reduction.
  6. Strong conflict resolution and interpersonal skills.
  7. Background working in Mental Health and Addictions.
  8. BSW or a combination of education and work-related experience.
  9. Must have reliable transportation and a valid driver's license.
