Physical Therapist | Provides assessment and treatment of patients requiring physical therapy services within the home care setting. Is a resource to the field staff in the area of community rehabilitation and individualized rehabilitation needs of the patient. Participates in the utilization review, quality improvement, and multidisciplinary conferences of the organization.

Qualifications for this position:

• Licensed to practice physical therapy within the state.
• Doctor of Physical Therapy degree preferred.
• Possesses a minimum of one-year physical therapy experience.
