Job description

Work Setting: Onsite at 12 Thorncliffe Park Drive
Salary: $22.75/hour

Competition: (2025-186-CC) Personal Support Worker (2025-186-CC) Employment Type: PSW Hourly, Bargaining Unit (1 Vacancy) Work Hours: 35 hours/week M-F 3pm - 11pm

Program Overview – Community, Home & Cluster Care, Community Care
The Community Care Unit provides a holistic approach to care at home for seniors and caregivers who are vulnerable, at risk and/or marginalized. The unit provides a wide range of programs such as Assisted Living Services, Home & Cluster Care, Social Work, Meals-on-Wheels, Adult Day programs, and transportation, which enable seniors to be as independent as possible in their homes. The unit also provides a variety of socio-recreational/wellness programs which enhance the quality of life and promote the health of the older adult.

What You Will Do

  • Recognizes that she/he is assigned to work based on program needs, client preferences and PSW skill set as required.
  • Recognizes that services are client-focused and delivered in a professional and culturally sensitive manner.
  • Complies with all agency, unit and program policies and procedures as required.
  • Provides service to clients relevant to the program and as authorized by the Supervisor.
  • Complies with all health and safety policies and procedures.
  • Able to provide service based on physical demands of the job which require physical strength, stamina and ability to undertake frequent light to moderate lifting (10-25 kgs).
  • Reports any injury received at work to the Supervisor/On-call within 3 hours on the day of injury.
  • Confirms his/her work schedule as required.
  • Uses the Alayacare Mobile Application on their cell phone to clock in and out of their scheduled visits/shifts.
  • Presents a professional image to clients and the community.
  • Uses good communication and interpersonal skills to work effectively with program staff and clients.
  • Follows all cluster routines and client service plans as indicated.
  • Prepares meals as per the menu cycle and consults with clients to modify the menu.
  • Documents accurately in the appropriate client and program records.
  • Ensures there are sufficient groceries and supplies for use.
  • Responds to building service/maintenance issues appropriately.
  • Supports opportunities/activities for the clients to maximize and maintain their physical and mental abilities.

What Will Set You Apart

  • Comprehensive understanding of how to support clients in meeting their Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (iADLs).
  • Strong communication skills to ensure clear and effective interactions.
  • Ability to work collaboratively within a team.
  • Flexibility and sound judgment in assessing and responding to clients’ diverse needs.
  • Knowledge of gerontology issues, healthcare systems, and social services applicable to the demographic served.
  • First Aid and CPR certification is a valuable asset.

What You Bring to the Team

  • Personal Support Worker Certificate and NACC Certificate (if applicable).
  • Minimum one (1) year experience providing personal care to seniors and people living with disabilities.
