Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR) is looking for Registered Physiotherapists to provide services to our pediatric clients in their school and homes within the community. The position can be flexible, with working hours ranging from part-time to full-time depending on your desired client caseload size.

PT Responsibilities:
- Provide ministry funded, community-based Physiotherapy services within the pediatric client’s (infant to 18 years of age) place of residence, private school and/or through virtual services
- Provide physiotherapy assessment (through interview, standardized and informal assessment techniques) in the areas of gross motor skills, coordination, neurological, orthopedic, developmental, etc.
- Develop and implement goal-oriented treatment plans in collaboration with clients/caregivers
- Assign patient care plan, and provide supervision and monitoring of physiotherapy assistants when appropriate
- Effectively communicate information by providing education to clients/caregivers and linking to community agencies/resources when appropriate
- Complete and maintain current and accurate documentation related to assessment findings, care plans, progress, and discharge summaries according to CAR, the referral source, and College of Physiotherapists of Ontario standards

Requirements:
- Registered Physiotherapist with the College of Physiotherapists of Ontario (required)
- Experience working within a community setting is an asset but new graduates are welcome
- Ability to work independently and as part of an inter-professional team
- Ability to prioritize, manage self-scheduling, and meet reporting deadlines
- Excellent oral, written, and report writing skills
- Proficiency with computers
- Strong interpersonal and organizational skills
- Reliable transportation is required for this position
