Job Summary:

Be part of Baycrest’s interdisciplinary specialized geriatric rehabilitation team. This role will provide occupational therapy assessment and treatment services to the Complex Continuing Care unit, Slow Stream Rehabilitation, Musculoskeletal Rehabilitation High-Tolerance areas, Behavioral Neurology and as clinical operational needs require, may be re-assigned to other areas. Reporting directly to the Manager and Interprofessional Practice Chief for Occupational Therapy, Physiotherapy & Therapeutic Recreation, the occupational therapist will also communicate and collaborate with Clinical Unit Managers to facilitate optimum patient care delivery.

Responsibilities include but are not limited to:

  1. Conducting client assessments, treatment planning and intervention as well as some discharge planning for older adults in a complex continuing care environment, utilizing both direct and consultative models of service delivery.
  2. Playing a key role in clinical decision-making and client advocacy for clients who may have multiple medical &/or mental health issues, and be experiencing changes in cognition, deconditioning, neurological and/or musculoskeletal conditions.
  3. Involvement in multi-faceted discharge scenarios and working with complex support systems.
  4. Comfort assigning to OTA/PTA when appropriate.
  5. Participating actively in the teaching programs for occupational therapy and OTA/PTA students.
  6. Supporting a systematic, evidence-based approach to client care, program development and evaluation, and professional practice, in close collaboration with inter-professional team members, volunteers and the Occupational Therapy clinical team.

Qualifications include but are not limited to:

  1. Licensure in good standing with the College of Occupational Therapists of Ontario.
  2. Confirmed status as ADP authorizer preferred.
  3. Experience with an older adult population with multiple medical conditions and/or cognitive impairment as well as mental health issues.
  4. Excellent interpersonal, communication, and problem-solving skills.
  5. Basic computer skills – Meditech, MDS, Microsoft Word and Excel, NRS/FIM preferred.
