Occupational Therapist - Home Care Assessment

(Belleville, Brockville, Trenton, Hamilton, Windsor, Chatham, Brampton, Barrie, Sudbury)

Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR), is looking for Occupational Therapists to complete Independent Living Services (ILS) assessments throughout the province of Ontario. The ILS assessments are to be completed in the residential homes of injured workers. This is a casual part-time opportunity based on referrals.

The following types of assessments will be provided:

1. Personal Care Assessments
2. Home Accessibility Assessments
3. Mobility A and B assessments

All Occupational Therapists that are in good standing with the College of Occupational Therapists and who have a minimum of 3 years work experience are eligible. In order to perform mobility assessments, you will need to be an Assistive Devices Authorizer (ADP Authorizer).

• Referrals will be within your preferred geographic area
• You will be able to schedule the assessments in collaboration with the client at your convenience
• You will be paid a premium rate
• In addition to compensation, you will be paid for mileage, travel, and training time
• You may accept or decline the work based on your availability
